Complete New Patient Forms
Save time at your first visit by filling out our new patient forms in advance! We have our forms available for download here if you would like to complete them at home and simply bring them with you to your appointment.
Download FormsRather wait? No problem! We just ask that you please arrive around 10 to 15 minutes early for your scheduled visit so you can have time to complete the forms at our office.
Planning Your First Visit
We recommend allotting around an hour to an hour and a half for your first visit. If you will be using dental insurance to pay for your care, we ask that you please bring your insurance information or card so we can make sure our files stay up-to-date.
This initial visit is a chance for us to get to know you and your dental needs, and it’s an opportunity for you to become familiar with our team and our office as well. This is also when we’ll sit down with you to come up with a personalized treatment plan. We welcome you to share anything that’s currently bothering you about your smile or that you’d like to change. We’ll recommend next steps and give you cost estimates so you can decide how to move forward with your dental care.
